When Symptoms Come and Go: The Role of Holter Monitoring


Intermittent arrhythmias can be difficult to diagnose because symptoms often aren't present during a routine ECG. Patients may report palpitations, dizziness, lightheadedness, unexplained fatigue, or near-syncope that occurs sporadically.


Holter monitoring helps correlate symptoms with cardiac rhythm, providing valuable information for diagnosing conditions such as atrial fibrillation, supraventricular tachycardia, and other rhythm disturbances.


Internal Medicine and Cardiology often work together in these cases, helping identify both cardiac and systemic factors that may be contributing to symptoms.


Case Insights

A patient in their early 60s was referred for evaluation of intermittent palpitations and brief episodes of lightheadedness. Initial examination and ECG were unremarkable, and symptoms were occurring only a few times per week.


A Holter monitor was arranged to further investigate the patient's symptoms. During monitoring, several episodes of paroxysmal atrial fibrillation were identified, correlating directly with the times the patient had documented symptoms.


Further assessment involved both Internal Medicine and Cardiology. Additional investigations helped rule out contributing systemic factors, while Cardiology provided guidance on stroke risk assessment and rhythm management options.
