Myocardial Perfusion Imaging (MPI) is not only a tool for diagnosing ischemia it also provides continuous ECG and clinical monitoring that can identify patients requiring immediate intervention.
During stress testing, significant ECG changes accompanied by symptoms such as chest discomfort or dyspnea should never be considered routine. Prompt recognition of widespread ST-segment depression with reciprocal ST elevation may indicate severe obstructive coronary artery disease requiring emergency assessment, even if symptoms and ECG changes resolve shortly after stopping stress.

Case Insights
A 77-year-old man undergoing pharmacologic stress myocardial perfusion imaging developed mild chest pressure and dyspnea during Persantine infusion. Continuous ECG monitoring showed diffuse ST-segment depression with reciprocal ST elevation in aVR and V1, suggesting extensive myocardial ischemia.
The test was stopped immediately and aminophylline was administered, resolving his symptoms and ECG changes within minutes. Despite this improvement, the high-risk ischemic pattern prompted urgent EMS transfer to the Emergency Department.
In hospital, elevated cardiac biomarkers confirmed NSTEMI. Coronary angiography revealed severe coronary artery disease requiring urgent revascularization.
