When hypertension remains uncontrolled, look beyond the number.

Blood pressure that remains above target despite multiple appropriately selected medications may reflect more than routine hypertension. Before further treatment escalation, it is important to confirm accurate office or out-of-office readings, assess medication adherence and review medications or substances that may be contributing. Once true resistant hypertension is established, evaluation for secondary causes and related cardiovascular or renal risk may be warranted.

Our new Hypertension Clinic will provide focused assessment of difficult-to-control hypertension alongside integrated management of CKD, diabetes, dyslipidemia and obesity.

Case Insights

A patient was referred to our clinic after several months of high blood pressure readings, despite taking three medications. During the visit, we reviewed their home readings, medication routine and recent lab results. Low potassium and protein in the urine suggested a possible underlying cause, so we arranged targeted testing, adjusted the treatment plan and scheduled closer follow-up to monitor both blood pressure and kidney health.

We are excited to announce Dr. Jocelynn Rajanayagam has joined our team, and is now accepting referrals with timely assessment.

With extensive experience managing complex, multi-system conditions, Dr. Rajanayagam will lead our new Hypertension Clinic, specializing in resistant hypertension and secondary-cause investigations, while also providing cardiovascular and renal risk management and expert care for challenging electrolyte abnormalities and unexplained proteinuria.

Dr. Rajanayagam completed an Internal Medicine Residency and Nephrology Fellowship at Henry Ford Hospital in Detroit and is licensed with the CPSO and board-certified by the ABIM. We are very excited to have Dr. Rajanayagam join our team to help deliver rapid community access for complex medical, cardio-metabolic, and renal-risk patients.
